Evidence-led corrections are attributable, reviewed and visible. Dated source records are not silently rewritten.

Report a suspected error

Give enough detail for us to find the claim and check it against the original evidence. Please do not include sensitive personal information, documents or account passwords.

Review process

  1. Record the report and preserve the affected version.
  2. Check whether it is a transcription error, source revision, classification change or interpretation dispute.
  3. Compare the claim with the exact primary source and any relevant methodology.
  4. Correct material errors promptly and recalculate dependent figures.
  5. Record the date, change and impact in the visible correction history.

Historic corrections

Material corrections are logged. Dated audit reports and official releases remain historical snapshots; a replacement source is linked rather than silently substituted.

The July 2026 correction pass replaced the NHS waiting-time baseline derived from rounded communications wording with the revised official time-series value.
